<span style="font-size: large;">More than 15 years later, after finishing advanced prosthodontics residency at the University of California in Los Angeles (UCLA), Dr. Steve Mark Gan was offered a $12,000 a month job in LA. He turned it down and decided to come back to the Philippines, sure that he was doing the right thing.</span></div>

<span style="font-size: large;">Today, Dr. Mark Gan is a respected leader in the Philippine dental scene. Only in his 30’s, he has single-handedly revived the industry and pioneered dental tourism in the country. His clinic, Gan Advanced Osseointegration Center (GAOC), is the Philippines’ most advanced dental implant center, and is also among the top 25 in the world. People of different cultures, race and color go to GAOC for their dental needs.</span></div>

I took out my life savings which I earned since I was a kid selling different things. That totaled $3,266. I used all of that to buy a second-hand dental chair. Then, I used my late grandmother’s dental clinic in Binondo,” Dr. Gan said.</span></div>

<span style="font-size: large;">“I did one cleaning. With that, I bought some material so I could do some pasta (filling). I worked 24/7. I accepted all kinds of patients, most of whom at that time were Chinese since my clinic was in Binondo,” he added. “Some say my success is so fast. But actually it’s not really an overnight success. I worked hard for many years. But it came with trade-offs, like I can’t play golf, I don’t go out and I missed so much family time.”</span></div>

<span style="font-size: large;">“Dr.Moy had a West Coast Center for Osseointegration. He doesn’t advertise. He didn’t use the word clinic, implant, surgery, dentist etc. But he is world famous. Osseointegration refers to bone and metal fusion which is what dental implants are. So, I followed in his steps,” Gan said.</span></div>

<span style="font-size: large;">In 1999, Dr. Gan partnered with some dentists and they set up a dental implant clinic in St. Luke’s Hospital.</span></div>

<span style="font-size: large;">“We revived the dental implant industry and we’re making waves in the dental political world. You see, dental implants were introduced in 1983. But the technology was very poor. There were more failures than success. People got scared of it,” Gan recounted.</span></div>

<span style="font-size: large;">“After a year, in 2000, I bought out my partners. I built my first Makati office, the GAOC at the Medical Plaza in Makati. I worked so hard to pay the bills and make ends meet. I even pawned my car to keep the practice,” he said.</span></div>

<span style="font-size: large;">Dr. Mark Gan is a visionary and a trend-setter. He wasn’t satisfied with just keeping the status quo. He wants to make a difference.</span></div>

My family was receiving dental care which was of very low standard compared to what I saw in the States. The standard of surgery was not up to par with international standards. So, when I came back home, I wanted to change all that,” he said.</span></div>

<span style="font-size: large;">“Most dentists would rather buy a car or something when they earn money. I invest in equipment and materials. We boast of having the most state-of-the-art equipment in the country. Nothing can come close to us. We’re always a step ahead ..not just 2 steps away.. but about a whole staircase away from other dental clinics,” Gan revealed.</span></div>

<span style="font-size: large;">Gan’s success is also due to his strong business acumen. “I was always in business growing up . You see, my parents never spoiled me. They taught us to work hard for our things, even our toys. That’s why even as a young kid, I learned to buy and sell.</span></div>

<span style="font-size: large;">Even while I was in elementary, I sold lemonade,” he said. “Then when we moved here, I sold calamansi juice. I also sold jeans and garments to my classmates.”</span></div>

<span style="font-size: large;">Gan wanted to take up business in college thinking medical practice will take some time. His father then convinced him to take up dentistry. After earning his degree, he went to the US for training in USC and UCLA.</span></div>

When it was already 11pm and the loan shark was not yet giving me the money I needed, my best friend offered – siya na lang daw magpapahiram sa akin, umuwi na lang kami. I then found out who my real friends are. Some people gave me empty promises and left me hanging.”</span></div>

<span style="font-size: large;">While so many dentists and doctors are immigrating abroad as nurses, Gan plans to stay in the country. His vision is to build a chain of dental centers which will give US-quality dentistry at an affordable price to the masses. He has been traveling to US, China, Europe, and parts of Asia to look at possibilities for putting in the right ingredients for the dental offices he plans to set up.</span></div>

<span style="font-size: large;">After decayed or broken areas of the tooth are corrected by the dentist, an image (scan) is taken of the prepared tooth and the surrounding teeth. This image, called a digital impression, draws the data into a computer. Proprietary software then creates a replacement part for the missing areas of the tooth, creating a virtual restoration. This is called reverse engineering. The software sends this virtual data to a milling machine where the replacement part is carved out of a solid block of ceramic or composite resin. Stains and glazes are fired to the surfaces of the milled ceramic crown or bridge to correct the otherwise monochromatic appearance of the restoration. The restoration is then adjusted in the patient’s mouth and cemented or bonded in place.</span></div>

<span style="font-size: large;">Elleven say they can create the porcelain veneers on-site in minutes - seven, to be precise - using a system called CEREC, which utilises computer-aided design and computer-aided manufacture.</span><br />

<span style="font-size: large;">The process involves scanning the mouth with 'a highly accurate specialised camera' that creates a virtual 3D image of the scanned mouth on a computer.  </span><br />

<span style="font-size: large;">Using this image, teeth suitable for having veneers or crowns are selected on the screen, and various design tools are used to create a virtual porcelain restoration. </span><br />

<span style="font-size: large;">This is then used to make a porcelain block shaded to match the patient's teeth to create the porcelain restorations on-site. </span><br />

<span style="font-size: large;">These porcelain restorations are then characterised and glazed in a porcelain oven, similar to glazing pottery.</span><br />

<span style="font-size: large;">These are then immediately bonded to the existing teeth and within a few hours a patient can have their smile transformed, according to the clinic.</span><br />

<span style="font-size: large;">Dr. Amparo Cabugwason Hernandez, who goes by her short name “Ampy,” hails from the Queen City of the South – Cebu City, Cebu, Philippines. She was born on May 12, 1932, a third child in a brood of six children of humble parents: Pascuala Guangco of Cebu City, a stay home mother and Teodulfo Cabugwason of Baybay, Leyte, an elementary public school principal and teacher. As a little girl, Ampy was shy, simple and gentle. She was close to her father and always eager to learn new things. She was always a diligent and serious student that it didn’t surprise her family to see young Ampy reap honors and awards early in her student life.</span></div>

<span style="font-size: large;">Ampy is married to Alberto Hernandez, a retired worker from the Illinois Department of Revenue where he worked for 25 years. Together, they are blessed with 5 children: Jose (deceased), Jonathan, a City of Chicago policeman, Jocelyn, a pastry entrepreneur, Jericho, a graphic artist and designer, and Jerome, a City of Chicago policeman. Ampy and Bert now have 5 beautiful and adorable grandchildren, all girls whom they visit frequently and spend a great deal of time with during their visits.</span></div>

<span style="font-size: large;">Going to college, Ampy desired first to become a Medical doctor, studying at the University of the Philippines in Cebu. However, upon reaching two years of Pre- Medicine, her destiny changed drastically when her father died. She knew then that she could not meet the financial demands of medical studies, and decided to shift course to Dentistry. She went to Manila’s Philippine Dental College, presently known as DeOcampo Memorial Schools, a pioneer among dental colleges in the islands. Her struggle as a student was one of extreme frugality due to limited financial resources. To get by, she had to ask help on a regular basis from her siblings. She was a conscientious student who knew how to work her schedule out to accommodate her other interests, such as writing, even while dealing with the rigorous demands of dental studies. A regular contributor to the “Philidens”, a college periodical, she once became writer of the month. Ampy also loves to write poems during her spare time. She considers her poetry as an expression of her soul.</span></div>

<span style="font-size: large;">Through diligence and perseverance, Ampy graduated in 1956 and out of 454 examinees in the entire country, placed 2nd in her school during the National Board Exam, and 4th on a national level. Her steadfastness and forbearance finally paid off.</span></div>

<span style="font-size: large;">She worked for the Department of Health Services in the Philippines before migrating to the United States. Settling in Chicago, Illinois, she started working as a Dental Assistant until passing the Illinois Dental Board Exams licensure in 1977, all in one take. Soon after, she worked in the Board of Health where she spent 15 years serving the West Town neighborhood Health Center. Her compassion for the underprivileged families made her a favorite dentist of many families in the community. Dr. Hernandez also maintains a private practice at night, which continues on a part-time basis to this day.</span></div>

In all her volunteer work, Dr. Hernandez sought nothing in return. She was happy enough that she was able to help and respond to a deeper sense of calling. Community service has been part of her strong commitment to help and a manifestation of her passion for the love of God.</span></div>

<span style="font-size: large;">Being a member of the American Dental Association (ADA), Illinois State Dental Society (ISDS), Chicago Dental Society (CDS), Dr. Hernandez has a wide span of social network. She is also a member of the National Association of Filipino Dentists in America (NAFDA, which has bases in New York, California, Chicago and other states. She was once elected as their national secretary for 2 years. Her colleagues often call her a “workaholic”, a lighter expression of their awe and appreciation of her work ethics and great attitude.</span></div>

<span style="font-size: large;">Additionally, Dr. Hernandez is a member and one of the founders of the Philippine American Dental Society of the Midwest (PADSM), a Chicago association of Filipino Dentists where she served as President in 1989-1990. Only recently, during its 36-year anniversary last September 2009, she was honored with their prestigious award of excellence for her achievements.</span></div>

<span style="font-size: large;">Dr. Hernandez’s significant contributions to her profession were highlighted by commendations, recognitions and appreciations from different sectors of government agencies and societies, such as one coming from the Philippine Dental College (PDC) in the Philippines, Department of School Health Services (DSHS) in the Philippines, the Chicago Board of Health (CBH) in West Town neighborhood Health Center in Chicago, Management Care Dental Systems (MCDS) in Chicago, The Chicago Public School (CPS), The Albany Park Community Center (APCC) in Chicago, the Philippine American Dental Society of the Midwest (PADSM), and from the Illinois House of Representatives – Esther Golar of the 6th District.</span></div>

This could be explained by the increased level of fluoride in drinking water, toothpaste and mouth rinse as well as the improvement in society’s hygiene 1-4. With the increasing consciousness of people toward their oral and dental hygiene, expectancies from the dentist for early caries detection has grew. On the other hand it has been proven that the local equal application of fluoride has no effect on all dental surfaces and doesn’t lead to reduction of caries in dental fissures, in such a way that occlusal caries constitutes more than 90% of caries in children 5,6.  </span></div>

<span style="font-size: large;">In addition researches have showed that caries detection in pits and fissures is harder. One of the reason mentioned, is the insolubility of the enamel reinforced by fluoride at the dental surface which results in no observation of decay in it, while in the layer beneath there might be some caries 5,7,8. Also, different organizations and institutions have mentioned reasons such as technology progress and decay detection equipment, which even capable of detecting caries in their initial stages. This resulted in less expense for patients and considerable improvement in tooth life span.</span></div>

<span style="font-size: large;">DIAGNOdent is used as a complementary tool beside visual examination for diagnosis of occlusal caries. DIAGNOdent decay detection is based on the principle that when Diode laser with 655nm wavelength is irradiated on dental surface, it is absorbed by metabolites of intraoral bacteria and these metabolites emit a red fluorescence. </span></div>

<span style="font-size: large;">This fluorescence reflected by the dental surface is indicated as a number between 0 and 99 on the screen of the device. Greater numbers are an indication of a greater decay area. Therefore Laser fluorescence provides a quantitative and non-invasive method for the diagnosis of dental caries 16-18. Although DIAGNOdent is considered a valuable tool in the diagnosis of decay, there are some concerns existing regarding its accuracy, for instance, there is no relation between the number showed by the DIAGNOdent and depth of decay 19.</span></div>
